The aim of the Community Treatment Team is to provide specialised evidence based skilled interventions to people with moderate to critical mental health needs that maximise their potential for recovery. The team works with people who have severe and enduring mental illness that often results in complex health and social care needs. How to get Referred - Referrals to Community Treatment Team are made to Single Point of Access, based at Hopewood Park, Sunderland by phone, letter, email, fax. Anyone can make a referral but we will seek service users consent to proceed with this. These are processed by administration staff for triage by Senior RNMH. Any complex referrals can be discussed in their daily MDT meeting. Referring information along with any other information available through RiO is discussed at triage and a decision is made regarding CTT intervention. Clients will either be offered MDT assessment or signposted to the most appropriate service.
